We want your switch to Evergreen Private Bank to be as hassle-free as possible. Our bankers would be happy to assist with completing the forms.

• Open an Evergreen Private Bank Account
• Transfer your Automatic Payments and Direct Deposits to your new Evergreen Private Account but completing the forms below.
• Complete the Account Change Form for all Direct Deposits and Automatic Payments. You will need to complete a separate form for each Automatic Payment and Direct Deposit.
• Close your Old Account
• Make sure to destroy old checks, ATM/Debit Cards and deposit slips once you receive your new checks from Evergreen Private Bank.
• Let outstanding checks and/or automatic payments process and clear. This process sometimes takes up to a week.
• Use this Account Closing Form to notify your financial institution of your desire to close your account.
